I just installed an contact form on my site wich works basically with only php, exept for a one line captcha refresh script (can be disabled).
All it takes is the following bit of code to be inserted in the content area of your page/post, and the captcha and fond folder in the site root (download below).

You will have to create an error and ok page for succes/failure of the validation.
Just look in the contact.php file in the download and set the urls straight.
The script relies on the download folder beeing in the root of the site, see the action url in the script above.
